Come out to Oak Street Health's ribbon cutting on Sept. 26th, 2024, at 11:00am, located at the Delco Plaza across the parking lot from Lowe's. We are a primary care doctor's office for adults on Medicare. With 12 years of history, Oak Street Health has reduced hospital admissions among their patients by 51%. We look forward to bringing our services and preventative, value-based care program to the York community. Hope to see you there!
