The successful MH Case Manager will assess individual mental health, drug and alcohol, and/or children’s needs of Shelter residents, and make referrals to appropriate services. The Case Manager provides supportive guidance and advocacy for the mental health needs of program participants, including crisis intervention; transport residents as needed to accomplish program goals; and design and instruct or arrange mental health and life skills workshops. Case Manager is responsible for preparing and maintaining required documentation and records, including client-centered goal plans, discharge plans, and aftercare records. This is a full-time salaried position. Minimum requirements: bachelor’s degree in social science. Must be at least 21 years old, possess valid driver’s license, insurable driving record, and have reliable transportation. PA State Police background check, Act 33 and/or FBI clearances.
