LPN - Clinical Education and Population Health Nurse Assistant

Job Description
  • Provide staff education assistance: 
  • Assist with development and maintenance of competency based education program for clinical staff at Family First Health, including Family Planning-related services; 
  • Contribute to staff training activities regarding competency based skills, clinical processes, family planning/ reproductive health, and identified community health topics;
  • Serve in a clinical consultant role with key organizational programs, such as Guided Care Coordination;
  • Collaborate with key partners to communicate training needs and opportunities; and
  • Provide staff health screenings and vaccines as needed.
  • Support Quality Improvement Department efforts to improve patient health outcomes on a population health level, including ongoing and periodic projects related to: 
  • Value-based and quality measure opportunities presented via healthcare payers and community partners; 
  • Population health-level patient education and wellness activities; and 
  • Provide feedback on improvement of clinical processes, measures. Recommend action plans as needed.
Requirements and Responsibilities:
• Minimum qualifications include an LPN license with at least one year of clinical staff training experience
• Current CPR certification
