Clinical Support Specialist – Home Health - FT

Description:
Working under the direct supervision of the Clinical Manager, the Clinical Support Specialist is responsible for assisting the Nursing Supervisor with review of OASIS and creation of 485s for proper patient management as well as discharge and transfer OASIS.  Works with the Director and Clinical Manager to complete tasks related to the QAPI Program and completes the clinical team scheduling in the absence of the Clinical Manager or as directed.  Serves as the backup for the Intake Specialist with completing patient referrals.

 
Requirements and Responsibilities:
[QUALIFICATIONS AND EDUCATION:
  • Must be a graduate of a Licensed Practical Nursing program.
EXPERIENCE: 
  • One year of clinical experience required, home health or acute care experience preferred.
  • Must possess excellent computer skills.
  • Must possess great attention to detail and be able to work on multiple projects simultaneously.
  • Excellent communication skills, both oral and written required.
LICENSURE, REGISTRATION, AND/OR CERTIFICATION:
  • Must possess current license as a Licensed Practical Nurse from the Commonwealth of Pennsylvania or made application for same.
  • Must possess valid driver’s license.
  • Must have the use of an automobile in good working condition and automobile insurance.
  • Must maintain current CPR certification.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • [Assists the Nursing Supervisor with the following:
  • Conducts admission, resumption of care, and recertification case conferences with field staff to ensure proper treatments, diagnoses, and utilization of services.
  • Reviews errors on OASIS and plan of care with the field staff to ensure the plan accurately reflects the patient’s condition and the individual needs are met.
  •  
  • Reviews OASIS on transfers and unexpected discharges as needed.
  • Reviews all pertinent data for review and signature by designated personnel.
  • Completes technical audit of chart at recertification and discharge to ensure physician orders are signed and visit notes are present for all visits confirmed in the software program.
  • Follows up on patients who were transferred to a facility but remain under home health care.
  • Attends appropriate meetings as scheduled and required to participate in coordination of patient care, receive agency updates, educational material, etc.
  • Maintains confidentiality of patient information and all Agency operations.
  • Answers telephone and directs calls to the appropriate recipient as needed.
  • Communicates with physician offices and field staff to ensure the patient receives all treatment in a timely manner.  If warranted, consults with hospital or medical facility staff and the admitting physician to determine patient needs and provide continuity of care.
  • Completes the referral and intake process for new patients as needed or when the Intake Specialist is not available.
  • Accepts tasks from the Director and/or Clinical Manager concerning the QAPI Program such as but not limited to;
  • Attendance and participation at the PIP (Process Improvement Project) meetings and reporting the minutes to the Safety Committee monthly.
  • Statistical collection of data and input
  • Tracking and recording of indicators
  • Active member of the QAPI Committee
  • Clinical Record Review
  • Incident Reporting data entered into the data collection system
  • Infection Control data entered into the data collection system
  • Completes the clinical scheduling of patient’s visits as needed, paying attention to continuity of care and the patient’s plan of care.
  • SECONDARY DUTIES AND RESPONSIBILITIES:
  • Performs general office duties such as answering phone calls, typing, copying, filing, faxing, etc.
  • Performs patient visits;
  • Follows the written plan of care
  • Identifies patient care needs based on ongoing home visits and reports these to the registered nurse; updating the Plan of Care.
  • Complies with Home Health documentation/communication expectations.
  • Provides education to patient and caregivers regarding medical condition, medications and nutritional requirements.
  • Performs skilled nursing procedures as ordered by patient’s physician in accordance with Home Health policies and procedures and LPN Practice Act.
  • Documents patient and/or caregiver response to care, interventions and/or teaching; Documents patient progress toward outcomes of care.

  • Complies with Home Health policies and procedures.
  • Maintains confidentiality of all patient and agency information following HIPAA guidelines regarding patient information.
  • Demonstrates awareness of own behavior and its effect on own/agency productivity and effectiveness.

  • Demonstrates responsibility in the role of Clinical Specialist.
  • Participates in staff meetings, agency sponsored in-services and team meetings.
